CDSCO Directs Safety Updates for Ibuprofen and Aceclofenac Over Risk of ‘Fixed Drug Eruption’

NEW DELHI: The Central Drugs Standard Control Organisation (CDSCO), under the Drugs Controller General of India (DCGI), has issued directives to all State and Union Territory (UT) Licensing Authorities requesting manufacturers of Ibuprofen and Aceclofenac to update the drug labeling and safety literature. The regulatory directives mandate the inclusion of “Fixed Drug Eruption” (FDE) as a recognized adverse drug reaction (ADR) in the Prescribing Information Leaflets (PILs), package inserts, and promotional materials of formulations containing these two non-steroidal anti-inflammatory drugs (NSAIDs).

The circulars follow safety recommendations generated by the National Coordination Centre for the Pharmacovigilance Programme of India (NCC-PvPI) at the Indian Pharmacopoeia Commission (IPC), Ghaziabad. Following an assessment of Individual Case Safety Reports (ICSRs) and signal evaluation, the Subject Expert Committee (SEC) reviewed the ADR findings and recommended regulatory action to ensure standard safety warnings across all brand and generic versions of Ibuprofen and Aceclofenac marketed in India.

Targeted Drugs: Oral and systemic formulations of Ibuprofen and Aceclofenac.
Mandated Action: All drug manufacturers operating under state and UT jurisdictions must immediately revise product literature—including Package Inserts, Prescribing Information Leaflets (PILs), and promotional material—to explicitly list Fixed Drug Eruption (FDE) under adverse reactions.
Regulatory Authority: Directed by the Office of the Drugs Controller General of India (DCGI), CDSCO, Ministry of Health & Family Welfare.

What is a Fixed Drug Eruption (FDE)?

Fixed Drug Eruption (FDE) is a distinct cutaneous adverse drug reaction characterized by well-demarcated, rounded, or oval erythematous (reddish to violaceous) macules or plaques that recur at the exact same anatomical site upon re-exposure to the offending agent.

While FDEs are generally self-limiting upon immediate discontinuation of the culprit drug, repeated administration can exacerbate the severity of lesions or cause post-inflammatory hyperpigmentation.

Impact on Clinical Practice and Healthcare Providers

The directive aims to enhance patient safety and help healthcare professionals quickly identify adverse cutaneous reactions linked to widely prescribed pain relievers:

Prompt Drug Identification: Clinicians evaluating patients presenting with localized, recurring skin patches are advised to take a thorough medication history, specifically noting recent intake of pain relievers or fever reducers such as Ibuprofen or Aceclofenac.
Patient Counseling: Patients experiencing recurrent skin spots or skin reactions after taking pain medication should be counseled to stop usage and consult a physician promptly.
Manufacturer Compliance: State Licensing Authorities have been asked to ensure time-bound compliance from drug manufacturers under their respective jurisdictions to align product literature with updated CDSCO standards.
