FIR Lodged Against Pharmacist in Chandauli for Working Nearly 10 Years on Forged Documents

Chandauli – In yet another shocking case of alleged fraud in Uttar Pradesh’s health department, an FIR has been registered against a pharmacist for securing and continuing employment for nearly a decade on the basis of forged documents.

The accused, Rajesh Kumar Chaudhary, was posted as a pharmacist at the Additional Primary Health Centre (APHC) Bhupauli in Chandauli district (under the Ali Nagar police station area, Niyamatabad). He was initially appointed in Ballia district in 2016 and has reportedly been serving in the state health services ever since.

According to the complaint, Chaudhary obtained the pharmacist post using kutrachit (forged) documents and suspicious certificates. A departmental probe revealed serious irregularities, prompting the health authorities to lodge a police case. The fraud allegedly allowed him to draw a salary and perform duties without genuine authorisation for approximately 10 years.

Timeline of the Case

2016: Rajesh Kumar Chaudhary was first appointed in Ballia district.

April 16, 2026: Strict directives issued by the Director General of Health Services, Uttar Pradesh, ordering verification of such appointments.

April 18, 2026: Chief Medical Officer (CMO), Chandauli, directed the Medical Officer In-Charge (MOIC) of Chahaniya Primary Health Centre to initiate action.

April 2026 (post-April 18): FIR registered at Ali Nagar Police Station on the written complaint of Dr. Ritesh Kumar, MOIC, Chahaniya PHC.

FIR and Investigation

The case has been registered at Ali Nagar Police Station under relevant sections of the Indian Penal Code. Police have begun a detailed investigation into the authenticity of the documents, the recruitment process, and other connected aspects.

Inspector Ghanshyam Shukla, Station House Officer (SHO), Ali Nagar Police Station, confirmed:

“Based on the complaint by the in-charge Medical Officer of Chahaniya Primary Health Centre, an FIR has been registered against the accused pharmacist under relevant sections, and the police have initiated a detailed investigation into the authenticity of the documents and other related aspects.”

Health Department’s Stand

The Uttar Pradesh Health Department has taken a firm position. Dr. Ritesh Kumar, MOIC, Chahaniya PHC, stated:

“This FIR has been lodged as per the strict orders of the government, and the department will not tolerate any forgery at any level.”

Officials have made it clear that if the allegations are proved, strict departmental as well as legal action will be taken against the accused, including possible termination of service and recovery of salary drawn over the years.

Growing Concern Over Fake Appointments

This latest case in Chandauli comes close on the heels of a similar FIR registered against two pharmacists in Sonbhadra district who allegedly worked for nine years on fake appointment letters. The recurring incidents point to possible loopholes in the verification process during recruitment and transfers in the state’s health services. Senior officials have assured that a wider probe into such irregularities across districts may be ordered.

The accused pharmacist is currently under investigation. Police have not yet revealed whether he has been arrested or if any other individuals are likely to be named in the case.
